Description JOB TITLE: Population Health Coordinator DEPARTMENT: Population HealthLOCATION: Rochester Regional Health Riedman Campus, 100 Kings HighwayHOURS PER WEEK: 40SCHEDULE: Monday through Friday, Standard Business Hours SUMMARYThe Population Health Coordinator is a key member of the practice team who has an understanding of primary care practices and assists in the coordination of care required for optimal coordinated population healthRESPONSIBILITIESDocuments all patient interactions in a concise manner that is compliant with documentation requirements Conducts inbound and outbound calls including, but not limited to: patient scheduling, screenings, reminder calls, panel management Assists to schedule and coordinate patient transportation, follow-up physician appointments Utilizes various reports to track patient referrals and scheduling no-shows for patients in collaboration with practice staff Provides clerical and / or administrative support to clinical staff and managers for special studies, projects and reports Provides excellent customer service by serving as a resource to all internal and external customers Maintains knowledge of all health plan benefits and payor requirements,Call patients as designated by PCP or clinical staffVerify patients have been seen and follow up on no show patientsAssist all patients with barriers Ensure appropriate clinical information is communicated to clinical staff when there is variance from plan of careCoordinates other medical office assistants in outreach and report validation needsCloses Care Gaps MINIMUM QUALIFICATIONSValid NYS drivers' licenseMore than one year of related experience in a medical setting PREFERRED QUALIFICATIONSCurrent Medical Assistant certification or registration preferredHigh school graduate or GED equivalent preferredBasic computer literacy Knowledge of medical terminology Good communication and customer service skills Able to perform all Medical Office Assistant job functions and responsibilitiesPHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.PAY RANGE: $17.85 - $19.50 The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
