UAS -NY Assessment Nurse
A UAS-NY assessment nurse is a nurse who assesses pre-screened community members for eligibility in Managed Long Term Care plans. They complete an interview with a patient or guardian in-person in the patient's home and finalize a UAS assessment to determine eligibility for Long Term Care services. They also explain the services to the potential member and their family. The role is available in Per-Diem positions. Current Rate is $160.00 per assessment via tele-health and $180 per assessment in person.
