Certified Nursing Assistant (CNA)- $22 and up (depends on borough)

Licensed Practical Nurse (LPN)- $40 - $45/hour

Registered Nurse (RN)- $50 - $70/hour

Documents Required for all Facilities:

COVID-19 Vaccination card/proof of first dose
Current NYS license (CNA, LPN or RN)
Updated Resume
CPR Certification (if applicable)
Two Professional Reference Letters
Driver’s license or government-issued picture ID (work authorization if applicable)
Social Security Card
An up-to-date physical (within the last year) that must indicate: PPD within the last year or chest x-ray if positive PPD within last 2 years and Measles, Mumps, Rubella, and Varicella titers.
