1. Observes and reports changes in patient/resident and family’s condition, mental state and social behavior to medical and professional staff caring for patients/residents and participates in the assessment and/or care of patients/residents and families.
2. Assists with the physical care and personal hygiene of selected patients/residents, consisting of, but not limited to, changing simple dressings, giving enemas and taking vital signs; prepares patient/resident for meals and feeds them if necessary; turning and positioning.
3. Escorts ambulatory patients/residents and transports non-ambulatory patients/residents by wheelchair, stretcher, bed or stretcher bed, including those with intravenous apparatuses to all departments, clinics, diagnostic and therapeutic units, religious and recreation centers, and other areas within the facility and facility's affiliation hospital; and explains nature of trips to patients/residents.
4. Collects and delivers specimens, charts, X-Rays, regular and emergency requisitions for diagnostic tests and treatment; and performs other messenger duties as required.
5. Provide for comfort needs of patients/residents and assists them with all aspects and activities of daily living.
6. Must perform related duties as assigned by the Charge Nurse, i.e. monitoring of resident smokers etc.
7. Maintains clinical competence by attending continuing education programs. Attends annual mandatory in-service training.
8. Assists in meeting resident/family emotional needs.
9. Measures intake and output.
10. Participates in Interdisciplinary Team Conferences when needed.
11. Prepares patients/residents for examination by performing assigned procedures such as temperature, pulse, respiration, height and weight measurements and assists in the examination process.
12. Prepares patients/residents and/or examination room for exam, treatment and/or procedures; sets up and arranges examination room. Places or positions patients/residents on table, bed, stretcher, wheelchair, etc. using Marisa lifters or other lifting devices, if necessary.
13. Maintains, assembles and dismantles devices and equipment used in patient care. Prepares and tests equipment for safe operation and transports various equipment and supplies.
14. Changes linens and make beds. Keeps patient/resident care unit, utility, treatment, storage rooms, shower rooms, and recreational areas in a neat and orderly fashion.
15. Assists in the care, storage and do inventory of patient/residents' clothing and property.
16. Maintains, controls, completes, processes and files patients/residents’ charts, records, transportation and time records, appropriate forms and documents.
17. Assists in clinic, admission procedure and in regulation of visitors.
